Field Trip Requirements

The following documents MUST be submitted PRIOR to the field trip:

1) PO worksheet (field trip date, destination, and justification)

2) Form 437a Request for Student Travel

3) Roberts Hawaii School Bus reservation form

4) Student roster

The PO worksheet will only be posted if the request is submitted with all required documents and approved prior to the field trip date by the Principal.

The posted PO draft copy with attached bus reservation form will be emailed directly to Roberts and the requestor will be cc'd in the email.

Roberts will confirm the reservation by email notification.
