HEALTH ROOM DOES NOT DISPENSE NOR HAVE AVAILABLE THE FOLLOWING: Tylenol, Advil, Motrin, Visine, contact solution, cough drops or any other kind of over the counter medicaiton.



Hawaii State Law requires that all new students in Hawaii must have the following documentation by the First Day of School:

a Valid Tuberculosis (TB) Clearance:

  1. TB skin test must be done within 12 months before first attending school in Hawaii.
  2. Documentation of TB test must include:
  • dates of administration & reading
  • results in millimeters (mm)
    • 0mm to 9mm aacceptable for school entry
    • 10mm or greater awritten documentation (including date performed and clinic location) of a negative chest x-ray must be provided
  • Signature or stamp of Hawaii Department of Health, or a US licensed MD, DO, APRN, or PA clinic

If NO TB clearance, student cannot enter school on first day

a Completed F14 - Student Health Record:

  1. Physical Examination must be done within 12 months prior to first attending school in Hawaii and signed by a US licensed MD, DO, APRN, or PA.
  2. Immunization requirements:
  • Grades K – 12:        5 DTap, 4 Polio, 3 Hepatitis B, 2 MMR and 1 or 2 Varicella (2nd dose of Varicella is required if 1st Varicella was given on or after 13th birthday)
  • Must be signed or stamped by a US licensed MD, DO, APRN, PA or clinic

Students who have not completed the physical examination and immunizations by the first day of school may be allowed 3 month provisional entrance only with a medical appointment card from a health care provider. If NO written evidence, student cannot enter school on the first day
